In order to reset your password, you must verify your credentials by entering the email address and date of birth which is linked to the account.
If you have multiple accounts registered to the same email address, please enter the date of birth of the individual which corresponds to the specific account you are resetting. To ensure accounts remain linked, the same email address and password must be used.
